The Delivery System Reform Incentive Program (DSRIP)


The Delivery System Reform Incentive Program (DSRIP) is the main mechanism by which New York State will implement to restructure the health care delivery system by reinvesting in the Medicaid program with the primary goal of reducing avoidable hospital use by 25% over 5 years. One of the components of this program is to measure patient behavior knowledge, and engagement in individual healthcare decisions. Measurements will be conducted through a 10-question Patient Activation Measure (PAM) survey. Non-utilizing and low utilizing Medicaid members, the uninsured, undocumented, and those who want to learn more about self-managing will be eligible to take the survey. The survey will identify where an individual falls within four different levels of activation, and based on the level, PHRC will provide the necessary resources and services (i.e., free Hepatitis B screening, breast cancer screening, diabetes prevention classes, etc.) the individual needs to confidently self-manage and achieve optimal health.
